The LPN will support daily nursing operations, coordinate with interdisciplinary teams, and ensure residents' individualized care plans are followed with excellence and empathy.

Responsibilities include direct nursing care, medication administration, monitoring residents, documentation, and collaboration with CNAs.

Qualifications

• Current and valid LPN license in Minnesota.
• Graduation from an accredited practical nursing program.
• CPR/BLS certification.
• Long-term care or skilled nursing experience preferred.
• Strong clinical, communication, and critical thinking skills.
• Compassionate, reliable, and team-oriented attitude.

Responsibilities

• Provide direct nursing care to residents per care plans.
• Administer medications and treatments as prescribed.
• Monitor residents and report changes to RN or physician.
• Document nursing interventions and responses accurately.
• Assist with assessments, admissions, discharges, and transfers.
• Collaborate with CNAs and staff to ensure high-quality care.
• Maintain a clean, safe, and supportive environment.
• Educate residents and families about health conditions and care needs.
• Uphold state, federal, and facility standards of care.
• Participate in ongoing training and quality improvement.
