Job summary

Life Care Centers of America’s San Luis Care Center in Alamosa, Colorado, is seeking a qualified Licensed Practical Nurse to join our skilled nursing team for short-term rehabilitation and long-term care.

The LPN will monitor patient conditions, provide nursing care, supervise CNAs, and maintain compliance with standards. Active Colorado LPN licensure and CPR certification are required; one year of skilled nursing experience preferred.

Qualifications

• Graduated from an accredited LPN program.
• Currently licensed/registered in applicable State with an active LPN/LVN license in good standing.
• CPR certification upon hire or obtain during orientation; CPR must remain current during employment.
• One year skilled nursing experience preferred.

• Monitor and report changes in patient condition.
