Frederick Living is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to residents and lead the neighborhood team. This role emphasizes person-centered care, supervises daily resident care, and supports staff development.

You will observe patient changes, coordinate with care providers and families, and ensure documentation and infections control best practices are followed. The position offers meaningful work and diverse daily responsibilities in a senior living community.

Qualifications

• Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania.
• High school diploma or equivalent.
• Prior experience in geriatric nursing preferred.
• BLS Healthcare Provider certification.
• Basic computer skills.
• Strong communication skills, both verbal and written.
• Ability to role model professional appearance and demeanor.

Responsabilités

• Observe and report changes in resident condition to the Nurse Supervisor or physician/physician extender.
• Communicate with Supervisor, ADON, DON, NHA, care providers, and family/POA as needed.
• Use evidence-based practices to ensure safe, high-quality care delivery.
• Assist in managing personnel through performance evaluation, Care Chats, coaching, counseling, education, and corrective action as needed.
• Complete all required documentation accurately and in a timely manner.
• Participate in resident transitions of care, care conferences, and care plan reviews.
• Collaborate with Cedarwood staff to ensure optimal resident outcomes.
• Comply with Infection Control practices.
• Ensure hand-held communication equipment is maintained for effective call bell management and team communication.
• Attend and complete all required in-services, training, and team meetings.
• Abide by the organization’s code of conduct.
• Perform other duties as assigned
