Job summary

Apploi in Bayport, NY is seeking a Licensed Practical Nurse (LPN) to join our skilled unit and long-term care team. You will provide direct resident care, administer medications (including IVs when required), monitor vital signs, and collaborate with physicians, NPs, families, and other health care professionals.

This role includes supervising CNAs, delivering resident education, and participating in care planning.

Qualifications

• LPN license is required.
• Experience in SNF/ LTC settings preferred.
• Ability to care for residents and document care accurately.

Responsibilities

• Provide care for residents in the Skilled Unit and Long Term Care Unit.
• Medication administration, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcomes.
• Provide resident and family education.
• Communicate with physicians, NPs, families and other health care professionals.
• Supervise CNAs.
