Job summary

Pavilion of Logan Square is seeking a Licensed Practical Nurse (LPN) to join their team in Chicago, IL. The LPN will assume responsibility for a group of residents during a shift, providing licensed nursing care and reporting with physicians and the IDT.

Responsibilities include medication administration, vital signs, care plan reviews, and adherence to infection control standards. On-site role with opportunities for growth and comprehensive benefits.

Qualifications

• Current CPR Certification required.
• Possess a current LPN license in the State.
• 1 year of clinical experience preferred.
• Long‑Term care experience preferred.

Responsibilities

• Provides licensed care to residents as ordered by physician and in accordance with standards.
• Receives and transcribes physician orders to charts and MARs.
• Prepares and administers medications and treatments as ordered.
• Maintains clean and sanitary work areas and follows safety protocols.
