Job summary

Holmes Lake Rehabilitation & Care Center in Lincoln, Nebraska is seeking a Licensed Practical Nurse (LPN) to deliver direct nursing care on a 6P-6A shift. You will assess resident needs, administer medications, monitor vital signs, and collaborate with the nursing team to support excellent resident outcomes.

This role requires Nebraska LPN/LVN licensure, graduation from an accredited program, CPR/BLS certification, and strong communication.

Qualifications

Valid LPN/LVN license in Nebraska.
Graduate from an accredited LPN/LVN program.
CPR/BLS certification current; state regulations may require additional certifications.

Responsibilities

Provide direct nursing care to residents and administer medications as prescribed.
Monitor residents' vital signs and report significant changes to the Charge Nurse or DON.
Assist in development and implementation of individualized care plans.
Maintain accurate documentation and communicate with residents, families, and team.
Adhere to facility policies, infection control, and safety protocols.
Provide emotional support and education to residents and families.
Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated care.
