Crescent Care of Elgin is seeking a Licensed Practical Nurse (LPN) to join their team. The LPN assumes responsibility for a group of residents during a shift, administering medications, recording vital signs, and coordinating with CNAs to deliver quality care.

This full-time role offers daily pay, competitive compensation, and a comprehensive benefits package as listed in the posting. Current Illinois LPN license and CPR certification required; one year of long-term care experience preferred.

Qualifications

• Current CPR Certification required.
• Licensed to practice in the State as an LPN.
• 1 year of long-term care experience preferred.
• Ability to administer medications and IV therapy within license limitations.
• Document patient care accurately and timely.
• Maintain clean and safe resident environment.
• Communicate effectively with residents, families, and team.
• Follow physician orders and care plans.
• Participate in shift reporting and bedside rounds.
• Commitment to infection control standards.

Responsabilités

• Provide licensed nursing care to assigned residents per physician orders and regulations.
• Administer medications and treatments as ordered.
• Record vital signs and monitor patient status.
• Transcribe orders to MAR and update charts.
• Coordinate with CNA team and supervise daily tasks.
• Ensure safety and maintain care environments.
• Prepare admissions, transfers, or discharges forms.
• Participate in care plan reviews and IDT meetings.
