Job summary

WeCare Centers at Foltsbrook Care Center in Herkimer, NY is actively seeking dedicated LPNs to join our team. You will provide essential nursing care, monitor patient health, administer medications, assist with daily activities, and collaborate with the healthcare team to ensure quality resident care.

Responsibilities

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation in an accurate and timely manner.
• Supervise, administer and/or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Ensure the residents care standards are maintained.
• Report to the Director of Nursing.
