Charter Senior Living - Jefferson City, MO is seeking a compassionate LPN to join our Assisted Living and Memory Care teams. The role focuses on resident assessments, medication administration, care planning support, and close collaboration with families and physicians to ensure high-quality, person-centered care.

Responsibilities

• Provide nursing care and clinical support to residents.
• Assist with resident assessments and care planning processes.
• Administer medications and treatments according to physician orders and company policies.
• Monitor resident conditions and communicate changes appropriately.
• Maintain accurate clinical documentation and medication records.
• Partner with caregivers, physicians, and families to ensure quality resident care.
• Support resident dignity, comfort, and quality of life.
• Assist with compliance, infection control, and quality assurance processes.
• Support and mentor caregiving staff as needed.
• Foster positive relationships with residents, families, and team members.
