Job summary

Senior Resource Group is seeking a Licensed Practical Nurse or Registered Nurse to join our community health team. The LPN/LN will oversee resident care, medication management, and ensure compliance with physician orders and state regulations.

You will collaborate with healthcare providers, document using Yardi EHR, and support care coordination for move-ins, extended leaves, and transitions. A compassionate approach and strong teamwork are essential.

Qualifications

• Must be a Licensed Practical Nurse and/or Registered Nurse holding a valid nursing license in good standing.
• Minimum of one (1) year of experience in a senior living environment preferred.
• First Aid and CPR certification required.
• Must pass a criminal background clearance before beginning work.
• Proficient with Microsoft Office, eMAR and other related software.

Responsibilities

• Work as part of the team within the Health Services Department.
• Participates in and completes resident’s assessments to identify resident care needs.
• Develops and updates resident service plans which include person centered approaches and interventions to determine resident care level.
• Follows up on residents’ health conditions, and /or changes of condition with healthcare providers, residents and responsible parties.
• Communicates relevant resident information to healthcare providers, alerts community staff and responsible parties of care concerns and service plan changes.
• Performs nursing services as directed and in accordance with the Nurses Practice Act, and state regulations.
• Develops and maintains effective relationships and communication with residents, families, responsible parties and other healthcare providers.
• Collaborate with healthcare providers by reviewing weekly visit notes and ensuring continuity of care.
• Coordinate onsite lab diagnostic testing services and/or durable medical equipment as needed.
