Job summary

Avardis Health is seeking a qualified Licensed Practical Nurse (LPN) to join our team at Pennknoll Village in Everett, PA. We offer 12-hour shifts with both day and night options, and a competitive wage range with shift differentials.

In this role, you will develop and implement individualized care plans, supervise CNAs, and oversee safety programs to ensure the highest standard of resident care. A compassionate, detail-oriented professional with an active LPN license is required.

Qualifications

• Active LPN license in good standing.
• Experience in skilled nursing or rehabilitation preferred.
• Compassionate, detail-oriented, and dedicated professional.

Responsibilities

• Assist in developing and implementing individualized treatment plans for residents.
• Supervise and support CNAs, including participation in their evaluations.
• Oversee the Falls Program and Pressure Sore Program to ensure resident safety and well-being.
• Provide essential nursing services and direct patient care.
• Conduct and document thorough medical evaluations upon admission and throughout treatment.
• Perform additional duties assigned to support the best possible care for our residents.
