Job summary

The GreenFields Continuing Care Community is seeking a dedicated Licensed Practical Nurse to provide high-quality care to residents under RN supervision. You will support CNAs, follow individualized care plans, and participate in direct resident care with attention to accuracy and compassion.

Qualifications

• Two years' experience in SNF, acute care, or long-term setting.
• LPN license is required.
• CPR/BLS certification required; First Aid helpful.

Responsibilities

• Assess resident needs and update ISP plan of care.
• Administer and record treatments/medications.
• Clarify and implement physician orders.
• Document resident care and responses.
• Coordinate transfers and readmissions from hospital.
• Count and administer medications per orders.
