Job summary

The GreenFields Continuing Care Community is seeking a licensed practical nurse (LPN) in New York to administer nursing care under RN supervision. You will coordinate with CNAs, update care plans, and ensure accurate documentation of treatments and resident responses.

Qualifications

• Two years' experience in SNF, acute care, or long-term setting.
• LPN License required.
• CPR/BLS certification.
• First Aid Certification.
• Willingness to cover shifts as needed.

Responsibilities

• Assess resident care needs and contribute to ISP plans.
• Document care, medications, and response to treatment.
• Administer treatments/medications per physician orders.
• Communicate resident condition to physicians and responsible parties.
• Coordinate hospital transfers and readmissions as needed.
• Provide direct resident care and guide care partners.
