The Licensed Practical Nurse (LPN) provides professional nursing care for residents in long term care under the supervision of a registered nurse, advanced practice provider, or physician. Demonstrates knowledge of nursing principles and uses appropriate age-specific standards to meet resident needs.

The role includes medication administration, documentation of resident interactions and outcomes, and collaboration with an interdisciplinary care team.

Qualifications

• Graduate from an accredited practical nursing program with LPN licensure.
• An associate degree in practical nursing is preferred.
• Current unencumbered LPN license and ongoing CEU requirements.

Responsibilities

• Provide nursing care under supervision in long-term care.
• Administer medications and treatments as ordered.
• Document resident status, interventions and outcomes.
