Job summary

PruittHealth @Home is seeking a Licensed Practical Nurse Scheduler to coordinate patient schedules across referrals, admissions, and discharges, ensuring timely communications and adherence to SOPs. This role supports back-office operations and collaboration with field staff in a home health setting.

The ideal candidate has an active LPN license, 1+ year of nursing experience, and familiarity with Home Care Home Base (HCHB). A focus on scheduling, accuracy, and teamwork is essential.

Qualifications

• Current, active, and unrestricted LPN licensure in the state of practice.
• Minimum of 1 year of practical nursing experience.
• Experience with Home Care Home Base (HCHB).
• 1 year of Agency Operations Experience.
• 1 year of Scheduling.

Responsibilities

• Provide accurate, timely schedules for all patients from referral through admission and discharge.
• Participate in back-office daily operations as directed by BOM.
• Demonstrate flexibility and critical thinking to adjust schedules as needed.
• Ensure scheduling complies with SOPs and maintains visit productivity.
• Maintain staffing availability reports and share information with key partners.
• Notify primary care physicians of care provision changes and secure new orders.
• Assist with intake, verifications, and documentation review.

Responsibilities

1. Primarily responsible for providing an accurate, timely, and well communicated schedules for all patients upon referral through admission, and discharge from the agency.
2. Actively participates in agency back office daily operations as assigned by the BOM.
3. Demonstrates flexibility and use of critical thinking to address frequent changes that occur throughout the day requiring schedule adjustments as needed.
4. Demonstrates that field scheduling assignments follow agency standard operating procedures (SOPs) and provides and maintains visit productivity for various levels of partners in the agency.
5. Keeps up the Home Health staffing availability report by mid-morning daily to provide information to other key partners of the organization.
6. Notifies the primary care physician ( as appropriate ) of any changes noted during care provision and secures new orders as needed.
7. Other duties as assigned include, but are not limited to, intake, verifications, and documentation review.

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice
• Minimum of 1 year of practival nursing experience
• Experience with Home Care Home Base (HCHB)
• 1 year of Agency Operations Experience
• 1 year of Scheduling
