Job summary

Fairfield Community Health Center in Lancaster, Ohio is seeking a Rooming Associate LPN to support patient flow, rooming, vitals, injections, and EHR documentation. The role collaborates with providers to enhance care access and outcomes.

The ideal candidate holds an LPN license, has Basic Life Support, and experience in ambulatory primary care or similar settings, with familiarity in NextGen and MS Office tools. A strong team player with good communication is essential.

Qualifications

• High school diploma or GED.
• Completion of an accredited Medical Assistant program. Medical Assistant certification or registration required.
• Graduate of an accredited school of practical nursing required. Current LPN license registered with the state board of nursing required.
• Current Basic Life Support certification.
• 2 years of experience in ambulatory primary care or related office setting.
• 1 year of Electronic Health Record (EHR) experience preferred.

Responsibilities

• Prepare examination rooms for patients, keeping rooms neat, clean, and stocked.
• Interview patients to obtain a history, current medical information, chief complaint, and reconcile medications.
• Perform in-house procedures and/or testing under provider direction (venipuncture, injections, immunizations, EKGs, lab testing, etc.).
• Document all patient encounters accurately in the EHR.
• Assist providers and maintain compliance with care guidelines and metrics.
