Job summary

Green Acres Healthcare in Kentucky is seeking a dedicated LPN or RN Unit Manager to lead the nursing team and ensure high-quality, personalized care for residents. This leadership role blends hands-on nursing with supervision, care coordination, and clinical oversight.

The Unit Manager will mentor RNs, LPNs, and CNAs, monitor care plans, and collaborate with physicians and nursing leadership to foster a positive, team-focused environment.

Qualifications

• Current LPN or RN license.
• At least 1 year of experience in a Long-Term Care setting preferred.
• Knowledge of Matrixcare is preferred.

Responsibilities

• Monitors clinical systems to support quality resident care.
• Delivers basic nursing care per physician orders.
• Directs RNs, LPNs and CNAs on the unit to ensure high-quality care.
• Updates care plans based on resident condition.
• Coordinates with ADON/DON to resolve grievances related to resident care.
• Documents nursing notes timely and accurately.
• Administers medications and IV duties per care plan.
• Conducts rounds to assess resident conditions.
• Participates in admissions, transfers and discharges.
