Job summary

Mt. Pleasant Health and Rehabilitation seeks a dedicated LPN or RN Unit Manager to lead nursing operations in our long-term care setting. You’ll monitor clinical systems, supervise RNs, LPNs and CNAs, and ensure care plans are followed, with a focus on resident safety and quality care.

This full-time role requires an active state license and at least one year in LTC; Matrixcare knowledge is preferred. You’ll join a team that values respect, teamwork, and compassionate service.

Qualifications

• Current LPN or RN license in state.
• At least one year of experience as an LPN or RN in a long-term care setting preferred.
• Knowledge of Matrixcare preferred.

Responsibilities

• Monitors facility clinical systems to facilitate quality resident care according to laws, regulations, and guidelines.
• Delivers basic nursing care to residents as ordered by a physician and per care plans.
• Directs and develops RNs, LPNs, and CNAs to ensure high-quality care in line with regulations.
• Conducts rounds to assess resident conditions and clinical practices.
• Documents nursing notes in resident charts promptly and accurately.
• Performs medication administration and IV duties as per care plan.
• Monitors resident progress and suggests treatment adjustments as needed.
• Observes resident behavior, vital signs, and responses to treatment; reports findings.
• Completes resident incident reports upon changes in condition.
• Monitors skin health to identify risk of pressure sores.
• Promotes team environment with praise and recognition among staff.
• Monitors staff schedules and shift adherence; manages breaks and overtime.
• Assesses staff performance and provides corrective action when needed.

LPN/RN Unit Manager Job Duties:

• Monitors facility clinical systems to effectively facilitate quality resident care in accordance with laws, regulations, and facility guidelines.

• Successfully delivers basic nursing care to residents in accordance with care plans as ordered by a physician.

• Directs and develops RNs, LPNs, and CNAs working on the unit to ensure the delivery of high-quality care and services in accordance with all laws, regulations, and guidelines.

• Conducts rounds within the nursing department to assess resident conditions and clinical practices.

• Accurately documents nursing care notes in resident charts in a timely manner.

• Performs medication administration and IV duties in accordance with the care plan as ordered by a physician.

• Monitors the progress of the residents and makes suggestions regarding treatment plans that may further meet their care needs.

• Observes resident behavior, vital signs, symptoms, responses to treatment, and reports findings to nursing supervisors.

• Completes resident incident reports upon identifying changes in the condition of residents.

• Monitors residents’ skin to identify the risk of pressure sores.

• Assists in promoting a team environment through effectively communicating praise and recognition with the RNs, LPNs and CNAs.

• Nursing Unit Manager monitors schedule adherence of staff for lunch and rest breaks, beginning and ending of shifts and overtime.

• Monitors job performance of RNs, LPNs and CNAs and provides corrective action as needed.

Unit Manager Requirements:

• Current LPN or RN license in state.

• At least one year of experience as an LPN or RN in a long-term care setting preferred.

• Knowledge of Matrixcare preferred.
