Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients.

Position Highlights

1. Pay: LPN hourly rate range - $30.00-$35; RN hourly rate range - $40.00-$45.00 (Per Diem)
2. Schedule: Fri, Sat, Sun, Mon overnights starting at 8p or 10p and ending at 6a or 8a
3. Area of Coverage: North, Central, South New Jersey

Responsibilities

1. Conduct initial home visits to assess patient needs.
2. Provide skilled nursing care following physicians' orders and medication profiles.
3. Document daily clinical notes and progress updates on the patient's clinical record.
4. Communicate with patients' physicians and other healthcare professionals to ensure appropriate patient care.

Qualifications

1. RN or LPN with at least 1 year of experience preferred.
2. Experience in home healthcare preferred.
3. Strong clinical judgment and critical thinking skills to make effective decisions, with adherence to company, state, and regulatory requirements.
