Job summary

Tacoma Nursing and Rehabilitation Center in Tacoma, WA seeks an experienced Resident Care Manager (LPN or RN) to lead resident care on a busy unit. This full-time role oversees rounds, documentation, and staff coaching to ensure high-quality, resident-centered care.

The ideal candidate holds an active WA license, CPR, 2+ years of nursing, and supervisory experience; long-term care experience preferred. On-site, 8-hour shifts, with an occasional on-call rotation and weekend coverage.

Qualifications

• Active LPN or RN license in WA
• Current CPR certification
• Minimum 2 years of nursing experience
• Case management or supervisory experience preferred
• Strong interpersonal skills with ability to guide and support team
• Ability to work under pressure, manage time effectively, stay organized
• Experience in long-term care or skilled nursing facility preferred

Responsibilities

• Conduct daily rounds to monitor resident condition and staff performance
• Provide coaching and/or counseling to staff as needed
• Review documentation for accuracy, compliance, and care quality
• Assist with emergencies, changes in condition, and communication with physicians
• Participate in care conferences and quarterly reviews
• Ensure weights are obtained per policy and referrals followed
• Participate in weekly wound rounds and assist with related documentation
• Take part in monthly psychotropic reviews and follow up on team recommendations
• Track consultant recommendations and ensure documentation is completed
• Assist with resident admissions and discharges
• Investigate incident reports using root cause analysis (5 Whys) and implement corrective actions
• Step in as a floor nurse during staffing emergencies
• Participate in on-call rotation (approximately every 5 weeks)
