Advocate Healthcare of East Boston is seeking a compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team of dedicated healthcare professionals. You will administer medications, monitor vital signs, and contribute to individualized care plans while collaborating with an interdisciplinary team.

Formación

• Must hold LPN or RN license and be in good standing.
• Experience in long-term care and resident-centered care is valued.
• Ability to administer medications and develop individualized care plans.

Responsabilidades

• Provide direct patient care, including administering medications, performing vital signs, and developing individualized care plans
• Collaborate with interdisciplinary teams to deliver comprehensive care and achieve patient goals
• Assess patient needs and develop strategies to address physical, emotional, and social needs
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
• Document patient information accurately and thoroughly in accordance with facility policies and regulations
• Participate in quality improvement initiatives to identify areas for improvement and implement evidence-based practices
