• Develop and implement individualized care plans in collaboration with other healthcare professionals
• Collaborate with other team members to ensure seamless delivery of care and services
• Maintain accurate and detailed records of resident care and services provided
• Participate in quality improvement initiatives to enhance the overall quality of care and services
