Overview

Our team is growing, and Palmetto Family Homecare is looking for LPNs & RNs to join our team as Nurse Case Managers, managing the in-homecare of our clients within the TriCounty and Midlands areas.

This is a “work from home” position that requires travel in your personal vehicle to visit clients in home, assisted living facility and / or in the hospital.

Responsibilities

• We have FULL-TIME & PART-TIME positions available
• One to one patient care, assessments and goal setting (no skilled nursing involved)
• Developing the initial plan of care coordination
• Conduct Quarterly Service visits with clients (Initial Visit, 30 day, 60 day, 90 day and Re-Evaluation)
• Performing observations and assessments of patient condition and needs
• Documenting all patient care actions
• Collaborating in the identification of patient needs and / or problems
• Client / family education and completing assessment / supervisory paperwork
• Assisting aides with in-house skills training as needed
• Staff Cases with Care Services Team attend Team Meetings and Education In-Services as required
• Review of weekly service sheets, service plans and notes
• 90 day and year assessment evaluations

Requirements

• Working knowledge of Microsoft Office Suite, Computer Skills and Resources
• Possess strong assessment skills.
• Must be energetic, detail oriented, outstanding organizational skills and able to multi-task.
• Must have strong written and verbal communication skills
• Detailed Charting abilities through our EMR system (Axiscare)
• Previous Case Management (preferred and a plus)
• Must be a self-starter who enjoys working with the senior community, disabled adults & veterans.
• Minimum Qualifications:
• Graduate of an accredited practical nurse or vocational nursing program
• Currently licensed as a SC Licensed Practical Nurse or Registered Nurse.
• Complies with accepted professional standards and practice
• One-year prior professional nursing experience, homecare experience preferred
• Current CPR
• Valid driver’s license, car insurance, and access to an automobile for home visits to client member’s place of residence.
