Major Responsibilities
  1. Oversee the coordination and completion of Resident Assessment Instruments (MDS, CAA's, and Care Plans) in compliance with federal and state regulations.
  2. Monitor and document management of Medicare and Managed Care residents in collaboration with the facility team.
  3. Implement innovative, responsible healthcare practices to improve systems and achieve superior results.
Minimum Qualifications
  1. Current, unencumbered active RN license in the state.
  2. Experience in Skilled Nursing/Rehabilitation facilities preferred.
  3. At least six (6) months of experience as an MDS Coordinator.
  4. RAC-CT or RNAC certification preferred.
  5. Must be qualified, compassionate, and dedicated to quality care.
