Job summary

Collierville Health and Rehab is seeking qualified LPNs and RNs to provide compassionate patient care in Collierville, TN. You will collaborate with the healthcare team, administer medications, and maintain detailed records to support residents' health.

Qualifications

• Current RN/LPN license in the state.
• At least one year of LPN or RN experience in a long‑term care setting preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers medications and records prescribed treatments.
• Records patients' medical information and vital signs.
• Monitors and adjusts equipment and interprets electronic displays.
• Initiates corrective action for adverse symptoms and participates in discharge planning.
• Provides bedside care for a wide variety of medical patients.
• Changes dressings, starts IVs, and assists during examinations and treatments.
