Care Finders Total Care, a Modivcare Personal Care Service, is looking for an RN or LPN responsible for providing high-quality services to our home care private duty clients.

Position Details

• Pay: LPN hourly rate range - $35/hr; RN hourly rate range - $40/hr
• Schedule: Tuesday, Thursday & every other Saturday 8am-8pm (12 hours)
• Location: Kenilworth, NJ
• Benefits: Full-time benefits package (see below for details)

Responsibilities

• Conduct initial home visits to assess patient needs.
• Provide skilled nursing care following physician's orders and medication profiles.
• Document daily clinical notes and progress updates.
• Communicate with physicians and healthcare professionals to ensure appropriate patient care.

Qualifications

• RN or LPN with at least 1 year of experience preferred.
• Experience in home healthcare preferred.
• Strong clinical judgment and critical thinking skills.
• Ability to adhere to company, state, and regulatory requirements.
