About

The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs and other nursing personnel. To monitor the performance of CNAs, nursing and non-licensed personnel, provide education and counseling, perform disciplinary action as necessary, and complete performance evaluations. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our Center, and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times.

Job responsibilities

Administers medications and treatment in a timely manner and document same.

Documents all pertinent data in resident records.

Participates in collecting data from MDS

Identifies and reports clinical decline to Unit Manager or Supervisor immediately

Attends care plan conferences to provide input

Communicates plan of care to CNA

Assists with developing, implementing and maintaining care plans.

Participates in would and fall prevention programs.

Provides in-service to CNA's as necessary

Communicates with physicians and writes orders according to policy

Monitors current infection control practices by CNA's.

Maintains confidentiality of resident care information

Maintains optimal nursing practices through attendance and participants in in-service education.

Requirements

Must have NJ LPN License (Active)
