Job description

Join to apply for the LPN/RN Clinical Triage, PCP (Harwich) role at Cape Cod Healthcare.

Responsibilities include:

  1. Monitoring the Electronic Health Record nurse pool throughout the day and responding promptly.
  2. Triage incoming calls, messages, and In-Basket tasks, prioritizing urgent matters and scheduling appointments accordingly.
  3. Applying nursing judgment and standard triage guidelines to meet patient and population health needs.
  4. Assessing complex patient needs to ensure proper care transition.
  5. Collecting objective and subjective data systematically and assessing responses.
  6. Triaging and scheduling visits based on patient needs and available resources; referring for emergency and follow-up care.
  7. Providing clinical support, adjusting roles and priorities as needed.
  8. Answering calls and electronic messages promptly and courteously.
  9. Documenting all interactions accurately in the patient’s medical record.
  10. Educating patients about community services and collaborating with the care team to ensure continuous care.
  11. Performing clinical duties such as taking vital signs, inputting clinical information, and assisting with procedures.
  12. Supporting patients and resolving concerns as an advocate.
  13. Participating in meetings and trainings for ongoing improvement.
  14. Maintaining confidentiality and adhering to HIPAA regulations.
  15. Providing clerical support like scheduling, greeting, and medical record management.
  16. Flexibility to float to other practices as required.
  17. Verifying and implementing clinician orders for tests and procedures.
  18. Triage of walk-in patients per protocols.
  19. Maintaining communication with the healthcare team for safe and effective care transitions.

Qualifications include:

  1. Ability to communicate effectively in English.
  2. Graduate of an accredited Practical Nursing or Nursing program.
  3. Current licensure in Massachusetts as an LPN or RN.
  4. Basic Life Support certification.
  5. Strong interpersonal and time management skills.
  6. Proficiency with electronic health records and office technology.
  7. Preferred: at least two years of clinical experience.

Position details:

  • Purpose: Provide high-quality clinical support under supervision, ensuring excellent patient care.
  • Schedule: 40 hours/week, Mon-Fri, 8 am-4 pm, with occasional weekends and holidays.
