Responsibilities include:
- Monitoring the Electronic Health Record nurse pool throughout the day and responding promptly.
- Triage incoming calls, messages, and In-Basket tasks, prioritizing urgent matters and scheduling appointments accordingly.
- Applying nursing judgment and standard triage guidelines to meet patient and population health needs.
- Assessing complex patient needs to ensure proper care transition.
- Collecting objective and subjective data systematically and assessing responses.
- Triaging and scheduling visits based on patient needs and available resources; referring for emergency and follow-up care.
- Providing clinical support, adjusting roles and priorities as needed.
- Answering calls and electronic messages promptly and courteously.
- Documenting all interactions accurately in the patient’s medical record.
- Educating patients about community services and collaborating with the care team to ensure continuous care.
- Performing clinical duties such as taking vital signs, inputting clinical information, and assisting with procedures.
- Supporting patients and resolving concerns as an advocate.
- Participating in meetings and trainings for ongoing improvement.
- Maintaining confidentiality and adhering to HIPAA regulations.
- Providing clerical support like scheduling, greeting, and medical record management.
- Flexibility to float to other practices as required.
- Verifying and implementing clinician orders for tests and procedures.
- Triage of walk-in patients per protocols.
- Maintaining communication with the healthcare team for safe and effective care transitions.
Qualifications include:
- Ability to communicate effectively in English.
- Graduate of an accredited Practical Nursing or Nursing program.
- Current licensure in Massachusetts as an LPN or RN.
- Basic Life Support certification.
- Strong interpersonal and time management skills.
- Proficiency with electronic health records and office technology.
- Preferred: at least two years of clinical experience.
Position details:
- Purpose: Provide high-quality clinical support under supervision, ensuring excellent patient care.
- Schedule: 40 hours/week, Mon-Fri, 8 am-4 pm, with occasional weekends and holidays.
