Typical Responsibilities:
- Follow individual service plans; administer or supervise treatments prescribed by physicians.
- Make rounds to all residents, utilize service plans, and record visits on charts.
- Administer injections, perform testing procedures, and record vital signs.
- Document acute episodes or significant changes in resident status.
- Collaborate with Activity Director, DON, and Nurse Aides in activities planning.
- Prepare and administer prescribed medications, order refills, and chart accordingly.
- Chart on your shift following facility policies and regulations.
- Supervise serving prescribed diets and fluid intake; report issues to physicians or supervisors.
- Prepare incident reports and complete assessments and discharge documentation accurately.
- Supervise residents who self-administer medication following guidelines.
- Assist with daily activities such as medication reminders, bathing, grooming, dressing, and escort services.
- Perform other duties as assigned.
Qualifications and Skills:
- Hold a valid LPN or RN license.
- Ability to lift at least 51 lbs regularly and assist residents with physical needs.
- Fluent in English (reading, writing, speaking, understanding).
- Experience with residents with memory impairment is a plus, especially in memory care areas.
