Job summary

Crossroads Treatment Centers in Darlington, SC is seeking an LPN or RN to join our outpatient MAT team. Follow orders, dispense daily and take-home meds, coordinate care with clinical staff, and maintain accurate patient documentation.

Preferred candidates have an active LPN/RN license in good standing, 3+ years of nursing experience, phlebotomy skills, and CPR/BLS certification. Schedule Monday–Friday 4:30am–12:30pm; you will work with a small nursing team and receive comprehensive training.

Qualifications

• Licensed Practical or Registered Nurse license in the practicing state.
• License in good standing and active.
• 3+ years nursing experience preferred.
• Strong phlebotomy skills; experience drawing blood on collapsed veins is a plus.
• Active CPR or BLS certification preferred.

Responsibilities

• Follow policies and procedures to correctly identify patients and administer medication per medical providers' orders.
• Dispense daily and take-home medication at the dosing window; coordinate care with clinical staff for continuity of patient care.
• Maintain accurate patient documentation including assessing for impairment and ensuring accurate medication counts.
• Participate in nursing team meetings and cross treatment meetings; connect with patients with empathy.
• Support compliance with documentation practices and expectations.

Day in the Life of an LPN/RN Follow policies and procedures to correctly identify patients and administer medication per the medical providers orders. Dispense daily and take-home medication at the dosing window. Coordinate care with clinical and medical staff for continuity of patient care. Maintain and complete accurate patient documentation including assessing for impairment. Ensuring medication count is accurate and completed daily. Follow compliance policies and procedures around documentation practices and expectations. Participate in nursing team meetings and cross treatment meetings. Connecting with patients, offering true empathy and a listening ear to those struggling.

Education and Experience requirements Licensed Practical or Registered Nurse License issued in the state in which they will be practicing or a multi-state compact status license. License should be in good standing, unencumbered, and active. If applicable, disciplinary actions documented on any license will be reviewed on a case-by-case basis. 3+ years of nursing experience is preferred. Strong phlebotomy skills, experience drawing blood on collapsed veins is a plus. Active and current CPR or BLS certification is preferred.

Schedule Monday-Friday; 4:30am–12:30pm.
