Norfolk Health and Rehab in Norfolk, VA is seeking a qualified Licensed Practical Nurse (LPN) to join our growing team. The role focuses on delivering individualized nursing care and coordinating with CNAs to uphold our high standard of resident outcomes.

You will administer medications, monitor vital signs, and participate in admissions, transfers, and discharges while maintaining detailed care plans and resident records.

Qualifications

• Active Licensure as a Licensed Practical Nurse (LPN) in state of practice.
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsabilités

• Supervise, coordinate, and direct the work of nursing assistants (CNAs).
• Provide individualized nursing care to residents according to each resident’s plan of care.
• Assist in the admission, transfer, and discharge of patients.
• Administer prescribed medications and treatments as ordered by the physician.
• Explain procedures and treatments ordered to gain patients’ cooperation and allay apprehension.
• Observe vital signs to detect deviations from normal and gauge progress of the patient.
• Maintain patients’ medical records on nursing observations and actions taken.
• Record nursing needs of patients on the care plan to ensure continuity of care.
• Observe emotional stability and prepare patients for continuing care after discharge.
• Perform other duties as required to provide exceptional care to each resident.
