Distinctive Living in Warren, NJ, seeks a full-time LPN to join Mira Vie at Warren. You will assess resident health status and contribute to care/service plans under the Director of Health and Wellness, ensuring regulatory compliance and high-quality care.

Responsibilities include documenting condition changes, coordinating with care staff, training, reviewing orders, and supporting CQI initiatives. Strong communication and nursing supervision are essential.

Qualifications

• LPN/LVN licensure required and a high school diploma or equivalent.
• One year of nursing or caregiver experience, preferably in assisted living, home health or memory care.
• Minimum six months experience in a supervisory role.
• Proficiency in Microsoft Word and Excel.
• Ability to read physician orders and match to MARs.

Responsibilities

• Follows the services outlined in each resident's service plan and documents changes of condition.
• Communicates with the Director of Health and Wellness regarding resident needs.
• Participates in providing hands-on care and nursing tasks in support of the care plan.
• Provides clinical support and training to care staff.
• Assists in transcription of physicians' orders and review of new orders.
• Maintains required certifications and in-service trainings.
