Job summary

Distinctive Living in New Jersey invites applications for a full-time or part-time LPN/LVN. The role focuses on resident health assessment, care plan development, and regulatory compliance under the Director of Health and Wellness.

Responsibilities include monitoring residents, communicating needs to health staff, supervising care aides, and ensuring accurate MAR documentation, order transcription, and ongoing training, while adhering to HIPAA and safety standards.

Qualifications

• LPN/LVN licensure required.
• One year of nursing or caregiver experience preferred.
• Supervisory experience preferred.

Responsibilities

• Follows services in the resident's plan and documents changes per rules and policies.
• Communicates resident needs to the Director, Health and Wellness as needed.
• Performs hands-on care and nursing tasks and reviews care plans for regulatory compliance.
• Provides clinical support and training to care staff.
• Assists in transcription of physicians' orders and reviews new orders.
• Maintains required certifications and ongoing trainings for licensing.
• Ensures HIPAA, OSHA and bloodborne pathogen guidelines are followed.
• Fosters teamwork and supports safe, respectful resident care.
