Wellmore of Daniel Island in Charleston, SC, is seeking a Resident Care Coordinator - LPN to oversee nursing in our Assisted Living and Memory Care units. The role directs and evaluates resident care, ensures regulatory compliance, and coordinates medications and care plans on a 24-hour basis.

We value teamwork, growth, and compassionate care. The ideal candidate holds an LPN license, a 2- or 4-year degree, and at least two years’ healthcare experience; five years is preferred.

Qualifications

• Ability to read, write, speak, and understand English fluently.
• Proficient with Electronic Records and office equipment.

Responsibilities

• Maintain residents’ charts with admission documents, care plans and regulatory records.
• Direct and oversee Assisted Living and Alzheimer’s care, liaising with regulatory agencies.
• Administer medications, assess responses, and adjust nursing activities as needed.
• Lead staff meetings and implement nursing programs.
• Conduct daily resident rounds and initiate corrective action as required.
• Participate in in-service training and education programs.
