Delmar Center for Rehabilitation and Nursing is hiring a Licensed Practical Nurse (LPN) to join our care team. The role includes admissions, medication administration, and collaborative care with an interdisciplinary team in a long-term care setting.

Qualifications

• CPR Certification required
• Two years prior nursing experience preferred
• LPN license must be in good standing with the state

Responsibilities

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident
• Implements the patient/resident plan of care and evaluates the patient/resident response
• Directs and supervises care given by other nursing personnel in selected situations
• Maintains knowledge of necessary documentation requirements
• Maintains knowledge of equipment set-up, maintenance and use
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information
