Job summary

Delmar Center for Rehabilitation and Nursing is hiring a Licensed Practical Nurse (LPN) to join our evening shift team. You will be responsible for admissions, discharges, and ongoing resident care under state Nurse Practice Act supervision.

Responsibilities include administering medications, adhering to infection control policies, and collaborating with an interdisciplinary team to achieve optimal resident outcomes. Prior nursing experience is preferred and CPR certification is required.

• LPN license in the state is required.
• CPR certification is required.

Pay Rate: $28-32.80/hr

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents




• Medication requisitions with pharmacy and physicians




• Medication administration




• Following infection control, emergencies, and other center policies




• Collaborating with resident’s interdisciplinary team to provide the best outcomes




• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy




• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.




• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.




• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.




• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.




• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.




• Maintains knowledge of necessary documentation requirements.




• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.




• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State




• CPR Certification required




• Two Years prior nursing experience preferred
