Job summary

Senior Lifestyle is seeking a licensed practical nurse (LPN) to join a clinical care team in Illinois. You will administer medications, participate in admissions assessments, and help design care plans for residents in accordance with regulations.

Requirements include an active LPN license, CPR certification, and at least 1 year of senior-care experience. Expect daily tasks that emphasize safety, patient education, and strong teamwork within the community.

Qualifications

• Must have a Licensed Practical Nurse license current in the state of practice.
• CPR certification is required.
• Minimum 1 year of experience in senior care or related field.

Responsibilities

• Administer medications for residents in accordance with Federal & State regulations and Senior Lifestyle guidelines.
• Assist with admissions assessments and documentation for new residents.
• Develop and monitor acute and long-term care plans or service plans.
• Coordinate with clinical staff and director on wound or infection control tracking as needed.
• Provide health-related consultations and notify physicians or family of changes in residents’ health.
• Respond promptly to emergencies and maintain clear communication across departments.
• Deliver health education to residents and staff and participate in required trainings.
• Adhere to safety and disaster plans and protect resident confidentiality.

Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.

• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Understand roll in the Safety & Disaster Plan.
• Maintain confidentiality of residents’ and prospective residents’ information.
• Attend all required training, in-service, and staff meetings.
• Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
• Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers.
• May be responsible for discharges, assessments, and clinical documentation as needed.
• Adhere to all policies and procedures of Senior Lifestyle Corporation.
• Perform other duties as assigned.
