WeCare Centers is hiring Licensed Practical Nurses (LPN) at Foltsbrook Care Center in Herkimer, NY. The LPN will provide essential nursing care, monitor health, administer medications, and assist residents with daily activities.

Qualifications

• Valid LPN license in the State of NY is required.

Responsabilités

• Conduct thorough assessments of patients’ medical status upon admissions and throughout treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.
• Complete required documentation accurately and timely.
• Supervise, administer and/or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Report to the Director of Nursing.

Job Duties
