Responsibilities

Job Summary

The Home Care Licensed Practical/Vocational Nurse provides direct nursing care as prescribed by a physician to acutely and/or chronically ill patients in their homes. Responsibilities include recording clinical notes, coordinating patient care activities/services, and working under the supervision of a Registered Nurse or designated supervisor. All activities are performed in compliance with state laws, licensure/certification requirements, and agency policies.

Specific Duties

• Deliver high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
• Thoroughly document patient visits in the electronic medical record and visit verification system.
• Participate in care coordination, report patient needs and changes, attend case conferences, and communicate effectively with patients, families, and healthcare team members.
• Maintain patient confidentiality per agency policies.

Education and Experience

License Requirements:
Current LPN licensure in the state of practice.
Valid driver’s license, vehicle insurance, and access to reliable transportation.
Current CPR certification.

Experience Requirements:
Previous home care experience preferred.
