Responsibilities

Job Summary:

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care within the geographic service area. Works under the supervision of the Registered Nurse or designated supervisor, in compliance with state laws, licensure/certification requirements, and agency policies.

Specific Job Duties/Responsibilities:

• Provides high-quality clinical services within the scope of practice, following the plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record, utilizing the agency’s electronic visit verification program when applicable.
• Participates in care coordination, reports patient needs and changes, attends case conferences, and communicates effectively with patients, families, physicians, and team members.
• Maintains confidentiality of patient health information per agency policy.

Education and Experience

License Requirements: Must have current LPN licensure in Tennessee, a valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation, and current CPR certification.

Experience Requirements: Previous home care experience preferred.
