Responsibilities

Job Summary

The Home Care Licensed Practical/Vocational Nurse provides direct nursing care as prescribed by physicians to acutely and/or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care within the assigned geographic area. The nurse works under supervision of a Registered Nurse or designated supervisor, within the scope of practice defined by state laws, licensure requirements, and agency policies.

Specific Duties

• Deliver high-quality clinical services within legal scope, following the care plan and infection control procedures.
• Thoroughly document patient visits in the electronic medical record, complying with regulatory and payer requirements.
• Participate in care coordination, report patient needs and changes promptly, attend case conferences, and communicate effectively with patients, families, physicians, and team members.
• Maintain confidentiality of patient information according to agency policy.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.

Experience: Previous home care experience preferred.
