Job summary

Diversity Group is seeking a Licensed Practical Nurse (LPN/LVN) to join the Veterans Health Administration (VHA) services environment. The role involves ongoing patient care within a PACT team, medication administration, and patient education, with opportunities for EDRP support.

Active licensure and graduation from an approved nursing program are required. The position emphasizes teamwork, adherence to safety protocols, and coordination of care for Veterans across clinic settings.

Qualifications

• Active, full, current, and unrestricted LPN/LVN licensure in the United States.
• Education from an approved school of practical or vocational nursing upon program completion.
• General qualification requirements and licensure expectations for the LPN/LVN role are specified by the VA/VHA.

Responsibilities

• Provide ongoing, continuous care for Veterans as part of the PACT team.
• Administer prescribed medications and document patient responses.
• Support clinic workflow and patient care coordination.
• Educate patients and families on care plans, medications, and health maintenance.
• Assist in emergency situations and promptly report changes to RN/MD.
