Job summary

Rome Health in New York is seeking a Licensed Practical Nurse (LPN) to provide direct patient care and clinical support in our Primary Care Offices. The LPN collaborates with the care team to deliver safe, efficient, and compassionate care to patients in the outpatient setting.

Key duties include patient assessments, education, medication administration, and EMR documentation. NY LPN license and current BLS/CPR are required; infection control and HIPAA policies must be followed.

Qualifications

  • New York State license as a Licensed Practical Nurse (LPN).
  • Graduate of an accredited practical nursing program.
  • Current BLS/CPR certification.

Responsibilities

  • Perform patient assessments and document patient information.
  • Provide patient education regarding care.
  • Administer treatments, medications, and injections as ordered.
  • Assist providers with examinations and minor procedures.
  • Maintain accurate and timely documentation in the EMR.
  • Follow infection-control, safety, HIPAA, and organizational policies.
  • Maintain clean, organized, and properly stocked clinical areas.

Education

Position Summary

The Licensed Practical Nurse (LPN) provides direct patient care and clinical support in the Primary Care Offices. The LPN works closely with the entire care team and other healthcare professionals to provide safe, efficient, and compassionate care to patients within the Outpatient Primary Care office other duties include, but are not limited to, the following essential job functions:

  • Perform patient assessments and document patient information
  • Provide patient education regarding care
  • Administer treatments, medications, and injections as ordered.
  • Assist providers with examinations and minor procedures
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Follow infection-control, safety, HIPAA, and organizational policies and procedures.
  • Maintain clean, organized, and properly stocked clinical areas.
Minimum Qualifications
  • New York State license as a Licensed Practical Nurse (LPN)
  • Graduate of an accredited practical nursing program.
  • Current BLS/CPR certification.
Physical Demands

Combination of walking and standing. Heavy lifting, 45 pounds or over and positioning of patients is required. Light carrying under 15 pounds. Pushing, pulling, kneeling and bending required. Frequent reaching, some above shoulders, handling and fingering of instruments and equipment. Talking and hearing for communicating with other personnel and patients. Hearing to distinguish differences and variations of vital signs of patients. Near-visual acuity required to work with a charts and records and to observe patients to detect changes in patient conditions, to read gauges and dials on equipment. Color vision for perceiving changes in patient's skin color, color of medicines and solutions.
