Job summary

United Health Services (UHS) Windsor is seeking a Licensed Practical Nurse to deliver skilled nursing care under supervision in our primary care setting in Windsor, NY. The role supports RNs, physicians and other clinicians with direct patient care and documentation.

Day shift with full-time hours and a competitive hourly pay range. The ideal candidate will have NYS licensure as an LPN (or Limited Permit) and CPR certifications; familiarity with EHRs is a plus.

Qualifications

• Graduated an accredited school of Practical Nursing.
• Current New York State Practical Nurse License, a Limited Permit, or ability to practice within the 90 day grace period allowed by New York State guidelines.
• CPR certification preferred.

Responsibilities

• Provide skilled nursing care under the direction of an RN or physician.
• Implement care plans and monitor patient status.
• Document care and outcomes in the patient record.
