Job summary

IU Health in Muncie, IN is seeking a Licensed Practical Nurse to join our patient care team. The role involves performing and documenting assessments within LPN scope, obtaining vital signs, administering medications, monitoring patient status, and supporting admissions, discharges, and transfers.

The successful candidate will provide wound care, collaborate with the healthcare team, and adhere to nursing policies.

Qualifications

• Graduation from a nationally accredited licensed practical nursing program.
• Active LPN license in Indiana or NLC expected.
• Basic Life Support (BLS) certification required.

Responsibilities

• Perform and document patient assessments within the LPN scope of practice.
• Obtain and monitor vital signs.
• Administer medications (oral, topical, SC, IM) per policy.
• Monitor patient condition and report changes to RN or provider.
• Assist with admission, discharge, and transfer processes.
• Provide wound care and dressing changes.

Typical Responsibilities

• Performing and documenting patient assessments within the LPN scope of practice.
• Obtaining and monitoring vital signs.
• Administering medications, including oral, topical, subcutaneous, and intramuscular medications as permitted by policy.
• Monitoring patients for changes in condition and reporting concerns to the RN or provider.
• Assisting with admission, discharge, and transfer processes.
• Providing wound care and dressing changes.
• Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients. Collaborates with other members of the healthcare team. Evaluates response to interventions and patient status toward goal achievement. Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.
• Requires graduation from a nationally accredited licensed practical nursing program.
• Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license.
• Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
• Successful completion of orientation required.
• 1-2 years of relevant experience preferred.
