Overview

St. Luke’s Health System in Boise is seeking a Remote Full-Time LPN to join our Connect Nursing/Nursing Administration – Preventative Care Nursing Team.

We are looking for an experienced Licensed Practical Nurse to join our Ambulatory Centralized Clinical Resources team. This position is fully remote, allowing you to provide patient care and support clinics from home while making a meaningful impact on patient outcomes across our health system.

Candidates must hold an active Licensed Practical Nurse (LPN) license in the state of Idaho. Out-of-state compact licenses will not be accepted.

Must reside in Idaho, Oregon, Utah or Arizona to qualify.

What You Can Expect:

  • A schedule of Monday - Friday 8am-5pm OR Tuesday - Friday 8am-5pm.
  • Play a key role in preventative care nursing by supporting a population health initiative focused on closing care gaps for patients across the health system

  • Conduct pre-visit planning for Medicare patients, including health risk assessments and follow-up coordination

  • Perform chart clean-up and scrubbing, with a focus on identifying care needs and preparing for upcoming visits

  • Communicate directly with patients to navigate health concerns, appointment needs, and care plan questions

  • Provide education on lab draws and procedures, supporting patient understanding and compliance

  • Spend approximately 60% of your time reviewing charts and interacting with patients over the phone, helping them prepare for or schedule needed care

  • Engage in special projects that support clinic operations and improve patient outcomes, particularly in identifying and addressing care gap.

Qualifications:

Education

  • Graduation from a Licensed Practical Nursing Program

Certifications

  • Current BLS-CPR Certification from the American Safety and Health Institute, American Heart Association or American Red Cross
  • Current Licensed Practical Nurse(LPN) licensure in the State of Practice

Preferred Qualifications:

  • Minimum of 3 years of ambulatory nursing experience
