Northshore Home Health Care is seeking a Licensed Practical Nurse (LPN) to support outpatient clinics in a team-oriented ambulatory care setting. You will obtain patient histories, take vital signs, and assist with examinations and diagnostic procedures.

Responsibilities include preparing specimens, administering medications and vaccines, and assisting providers with minor procedures, while ensuring accurate documentation and patient safety.

Qualificações

Role:

Licensed Practical Nurse (LPN) supporting outpatient clinics in a team-oriented ambulatory care setting.

Responsibilities:

• Provide clinical support by obtaining patient histories, vital signs, and assisting with examinations and diagnostic procedures.
• Prepare and collect laboratory specimens, administer medications and vaccines, and assist providers with minor surgical and diagnostic tests.
• Ensure accurate documentation, maintain a clean environment, and facilitate effective communication between patients and providers.

Qualifications & Skills:

Graduate from a nursing specialty training program. New and experienced LPNs are welcomed. Ability to perform clinical tasks within scope of practice, maintain accuracy in documentation, and promote patient safety and comfort.

Key Duties:

• Support diagnosis and treatment, follow up on diagnostic tests, prepare patients for examinations, and ensure readiness of examination rooms.
• Execute and document patient communication, respond to patient inquiries, and support provider efficiency.

Position Details:

Full-time, 40 hours weekly, with a focus on delivering high-quality patient care in a clinical environment. Adherence to policies and procedures is essential.
