Qualifications

LPN license or nursing degree from an accredited program.
Must be graduate of approved LPN nursing program.

Responsibilities

Medication administration and treatments.
Assist with admission, transfer and discharge of residents.
Document resident care in medical records.
Educate residents and families about treatment and status changes.
Adhere to infection control and facility policies.
Consult with physician about plan of care and changes.
Maintain resident confidentiality and privacy.
Ensure DOH and facility standards of care.
Collaborate with interdisciplinary team to solve problems.
