Job summary

Creekside Center for Rehabilitation and Healing in Ashland City, TN seeks an LPN to join our post-acute care team. You will provide medication administration, assist with admissions and transfers, and document care in the resident chart.

Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.

Responsibilities

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents.
• Document resident care in medical records.
• Educate residents and families about treatment plans.
• Adhere to infection control policies and facility policies.
• Consult with physicians regarding plan of care.
• Maintain resident confidentiality and privacy.
• Ensure quality of care per DOH standards.
• Collaborate with the interdisciplinary team to solve problems.
