Job summary

Jefferson City Medical Group seeks a Licensed Practical Nurse to serve as Population Health Care Coordinator. This on-site role at 1241 West Stadium Blvd, Jefferson City, MO, focuses on developing patient-centered care plans and coordinating with the care team to improve health outcomes.

Responsibilities include patient support calls, maintaining current records, initiating TCM and CCM services, coordinating teaching, and guiding transitions of care while addressing gaps in care.

Qualifications

• Completion of an accredited Licensed Practical Nursing Program.
• Ability to communicate effectively with patients, families and staff.
• Experience or interest in patient-centered care and care coordination.

Responsibilities

• Develop, implement and update patient-centered plans of care with the care team.
• Conduct patient support calls to identify resources and education needs.
• Maintain patient records reflecting current healthcare needs and goals.
• Initiate TCM and CCM services as directed by providers.
• Identify learning opportunities and coordinate teaching on health management.
• Assist with transitions of care from acute settings to home and follow-up care.
• Support patient engagement to facilitate appropriate use of healthcare services.
• Perform medication reconciliation with patients and families.
